Required Documentation and a completed IRS Form 13614-C Intake/Interview and QR Sheet
Please bring the following documents for your tax preparation visit:

Photo Identification (government-issued) for taxpayer and spouse (if applicable)

If filing jointly, both spouses must be present to sign the tax return and e-file return.

All income statements: W-2, 1099s, 1099-R, Social Security Benefits SSA-1099, Unemployment 1099-G and other income sources (cash, tips, or other property/services)

Documentation for all deductions/credits (including qualified charitable contributions), energy, educator expense, student loan interest, etc.)

Social Security cards and/or ITIN cards for taxpayer, spouse, and all dependents

An Individual Identification Number (ITIN) assignment letter can substitute if you do not have a Social Security number (SSN) for the taxpayer, spouse, and all dependents.

Proof of foreign status, if applying for an ITIN

Birth dates for taxpayers, spouse, and all dependents.

Tuition Statement (1098-T) and related expenses from college or technical school

Self-Employed Taxpayers
IRS VITA program self-employed taxpayer(s) requirements:
- Have gross receipts summarized.
- Business expenses totaled less than $35,000.
- Car expenses include total miles driven, miles used for business, miles commuting and miles used for personal use. Vehicle expenses such as gas, oil, maintenance, etc., cannot be used to report car expenses. Car rental or lease cannot be for more than 30 days.
- Used the “cash” method of accounting.
- Did NOT have an inventory at any time during the year.
- Did NOT have a net loss from your business.
- Had only one (1) business as a sole proprietor, qualified joint venture or statutory employee
- Did not require any credit card or similar payments that included amounts that are not includable income
- Are not required to file Form 4562, Depreciation and Amortization, for this business
- Had no employees during the year.
- Do NOT deduct expenses for business use of your home.
- Do NOT have prior year un-allowed passive activity losses from this business.
- Do NOT have transactions involving virtual currency (bitcoins)
- Income IS NOT from a hobby or not for profit activity, bartering or professional gambling
If you do not meet the requirements, your 2024 tax return will need to be prepared by a paid preparer.

Alternative Scanning Methods
If you have a smart device (phone, tablet/computer), you can use Genius Scan app.
- Download Genius Scan app.
- Open the app > Place document on flat surface.
- Press the orange circle w/ + symbol to begin creating pdf file.
- Press the moving circle above scan to take a picture of document.
- Select the + symbol to allow adding more pages in the file.
- Select Done > Select Add Tag and Name Your Tax File

Print a PDF File:
To print to a PDF for Form 14446 and Form 13614-C, follow these steps:
- Open/Download the document or content you wish to print, and press Ctrl + P at same time
The Print menu will appear revealing several options. - Open the Printer or Destination drop-down menu (Click down arrow)
A menu of printer choices appears. - Select Save as PDF or Microsoft Print to PDF
In some applications, you must first click Save as File before you can select PDF as the saved file type. - Click Print
